All services are provided strictly in accordance with the laws, regulations, and healthcare guidelines of the State of Kerala and the Government of India, ensuring the safety, dignity, wellbeing, and legal protection of every individual under our care.
Our Role & Scope of Service
Strictly Supportive & Hospital/Doctor-Guided Care
Care Bridge Home operates as a supportive healthcare service provider, and our role is strictly limited to assistance, monitoring, education, and guidance.
Core Principle of Service Delivery
  • Our services are 100% aligned with the treatment plan and instructions of:
    • The treating Registered Medical Practitioner, or
    • The treating hospital/healthcare institution
  • We strictly follow the medical direction provided and:
    • Do not modify, alter, or deviate from the prescribed treatment plan
    • Do not introduce any alternative clinical/medical treatment care plan, treatment, or intervention
  • Our objective is to support and implement the care outcomes intended by the hospital or treating doctor, ensuring continuity of care after discharge or during home-based support
Nature of Services (Supportive Role Only)
Our role includes:
  • Assistance with daily care and activities
  • Monitoring general health condition (non-diagnostic)
  • Supporting prescribed care routines
  • Educating families on care practices
  • Providing emotional and practical support
We do not function as a hospital or independent medical authority and operate strictly within the legal scope of home healthcare services.
Medical Documentation & Communication Requirement
To ensure safe and compliant care:
  • We request families to provide:
    • Discharge summary (for post-hospital cases)
    • Current treatment plan and medical instructions
  • Where required, we may:
    • Seek consent from the family/client to communicate directly with the treating doctor or hospital
    • Clarify treatment instructions to ensure accurate and safe service delivery
  • Services will be delivered only based on available and verified medical guidance
Scope Limitation – Not a Substitute for Hospital Care
Care Bridge Home:
  • Does not replace hospital-based care
  • Does not provide emergency, intensive, or critical care independently
In case of any deterioration or emergency:
  • Will follow the emergency plan
  • The family will be informed immediately
  • Transfer to a hospital or appropriate medical facility will be advised
Transparency & Client Awareness
We ensure that clients and families clearly understand:
  • The exact scope and limitations of our services
  • That we strictly follow doctor/hospital instructions
  • That we do not provide independent medical decision-making
Our Commitment (What We Do)
  • Deploy qualified, trained, verified, and registered nurses and caregivers only
  • Ensure all clinical tasks are performed only by appropriately qualified and registered professionals
  • Follow the hospital/doctor’s treatment plan without deviation
  • Maintain proper documentation, reporting, and communication
  • Follow infection control, hygiene, and safety protocols
  • Protect client confidentiality and privacy
  • Coordinate with doctors, hospitals, and emergency services when required
Our Limitations (What We Do Not Do)
  • Do not diagnose or prescribe medications
  • Do not alter or deviate from the doctor’s treatment plan
  • Do not provide alternative or independent hospital treatment care plans
  • Do not provide advanced home care without:
    • Hospital/Doctor advise and permission to the client or family
    • Hospital/Doctor’s direction
    • Hospital/doctor treatment care plan
    • Family consent
    • Acceptance by Care Bridge Home
  • Do not replace hospital care in emergencies
  • Do not provide services beyond legal scope
Staffing & Clinical Responsibility
  • We do not assign unqualified or unregistered staff to perform any clinical tasks
  • Procedures such as:
    • Injections
    • Any clinical intervention
      are performed only by qualified and registered nursing professionals
  • Caregivers are assigned only for non-clinical supportive roles
  • All staff are deployed only after:
    • Identity verification
    • Reference checks
    • Police Clearance Certificate (PCC)
Ethics, Integrity & Responsibility
  • Commitment to honesty, transparency, and accountability
  • Clear communication of service scope and limitations
  • Zero tolerance for:
    • Negligence
    • Any forms of Abuse or misconduct
    • Smoking, Drug or alcohol use during duty
  • Respect for client dignity, culture, and personal preferences
  • Adherence to recognized care standards and ethical practices
Client & Family Responsibility
  • Provide complete and accurate medical information and documents
  • Share discharge summaries and treatment plans
  • Allow communication with doctor/hospital when required
  • Follow medical advice and treatment plans
  • Inform any changes in condition immediately
  • Use hospital/emergency services when required
  • Provide consent and cooperation for safe care delivery
