The aim of the care package is to provide seniors with a person centered care and support services to improve their functioning, independence and quality of life.

What is a Care Package?

A Care Bridge Home Care Package refers to a comprehensive and personalized set of services and support designed for seniors, people with disabilities, and individuals with various medical conditions.


This package provides health care assistance both at home and in residential care settings, ensuring continuity of care and seamless transitions between different levels of care.

 

Components of a Care Bridge Home Care Package include medical and clinical support services, allied health support services, rehabilitation services, hospital related services, personal care, household assistance, social support, respite care, nutritional support, and transportation.

 

These care packages are customized to meet the unique needs of each individual, ensuring they receive the appropriate level of care and support whether at home or transitioning to or from a residential care facility. The goal is to enhance quality of life, maintain independence, and provide continuous, comprehensive care.

The in-home Care package is a coordinated mix of services designed on the basis of a comprehensive assessment that helps the elderly person stay independently at home for a longer period of time. The in-home care packages are designed to assist seniors who want to live safely and independently in their own homes for as long as possible.

 

Care packages are also offered to anyone with a disability or dementia who needs coordinated care and support to stay at home.

Levels of in-home care packages

Care Bridge Home offers four package levels that reflect the range of care needs. Every level of the package is designed to deliver different levels of support.

 

A level one package is for those who have the lowest care needs, and a level four package is for those who have the highest or most complex needs. The outcome of the assessment will allow the evaluators to understand the person’s needs and to recommend the type of services that best suit for the person.

 

The assessment will also include caregivers or family members who are directly in contact with the elderly.

Care Packages are tailored to each individual’s needs based on a comprehensive assessment. The purpose of the assessment is to determine the individual’s specific needs and condition, and to design and provide an appropriate level of care. The assessment will focus on the following areas:

The assessment will focus on the areas of physical health, mental health, occupational health, nutrition, social and community functions, home management, family/family needs, family links, etc.

The geriatric assessment facilitates the diagnosis of medical conditions, development of treatment and follow-up plans, coordination of care management, and evaluation of long-term care needs and optimal placement. Specific elements of physical health that are assessed include nutrition, vision, hearing, fecal and urine continence, and balance.
